健檢大腸鏡剛做完沒幾年 為什麼還會得到大腸癌?

最近因為名人效應,門診常常有病患問:『剛做完健檢大腸鏡沒幾年,為什麼還會得到大腸癌?』要怎麼來預防?

前一陣子一位知名音樂人因大腸癌而過逝,大家都非常惋惜!姑且不論是否『健檢沒事卻腸癌』,但這種情形是否真的存在?一位有大腸癌家族史的李先生,就問到這個問題。


醫師:『大腸鏡檢查做完,醫師說沒問題,但在下次檢查前(如5年),卻發生大腸癌!』,這種情形確實偶而會發生,醫學上稱之為區間癌(interval cancer),發生率介於2~9% (平均為6%)。

李先生:為什麽會這樣?

醫師:區間癌的發生,有兩種可能原因:(1)患者不幸罹患非常惡性的癌症,進展非常快速; (2)進行大腸鏡檢查時,因某些原因造成瘜肉或早期癌沒有被發現。 其中,以第二種原因較常見。

李先生:為什麼大腸瘜肉或早期癌,大腸鏡檢查會沒有發現?

醫師:根據文獻統計報告,有高達40%的息肉,以及高達24%的腺瘤(會癌化的瘜肉),在一般大腸鏡檢查時沒有被檢查出來,造成這種情形歸納有三大可能原因:(1)檢查前清腸準備工作,做得不夠確實,使得糞便沒有清除乾淨,阻擋了大腸鏡檢查視野,當然就不容易發現瘜肉;(2)醫師檢查時,回拉速度太快,觀察時間過短,使得容易漏失瘜肉,故國內外醫學會均建議,醫師操作檢查時,大腸整體觀察時間需大於6分鐘;(3)大腸本身就有許多皺摺,有些瘜肉會躲在皺摺後方,或被皺摺蓋住(如下圖),醫師就無法發現。所以,要提升大腸鏡檢查效率和品質,醫師和民眾都要一起努力做好檢查前和檢查中的任務。


李先生:既然大腸有那麽多皺摺,要怎麼避免瘜肉被皺摺蓋住,因而漏失掉?

醫師:現在臨床上有開發出一種大腸鏡的輔助耗材-內視鏡套帽(endocuff),我稱之為『小刺蝟』,因為此裝置具備八個柔軟的塑膠突出物,可將大腸皺摺攤平,讓瘜肉可以露出,醫師就可以更簡單地發現瘜肉,並加以切除,減少後續罹患大腸癌的風險,根據義大醫院及國外的研究顯示,使用『小刺蝟』可以提升大腸息肉的檢出率約10%;過去國外的研究也顯示,提升1%的腺瘤檢出率可以降低3%區間癌(interval cancer)的發生及5%的死亡率,所以內視鏡套帽(endocuff)是一種很推薦的輔助工具,建議大腸鏡健康檢查時,可以自費加裝使用。

李先生:好的,那我要確實做好清腸的準備工作,醫師就請您幫我用小刺蝟仔細看囉。

醫生說胃鏡檢查正常 我到底有沒有胃食道逆流?

門診常常有患者問:『我做過胃鏡很多次,醫師都說正常,我到底有沒有胃食道逆流?』


今天一個年輕的李小姐,遇到相同的問題,她平時經常有胸口悶痛、喉嚨異物感,斷斷續續已約兩年多,在醫院和診所接受過三次胃鏡檢查,做到都有點害怕了,醫師每次都說正常,服用一些藥物,也時好時壞,到底她是不是胃食道逆流呢?


醫師:胃食道逆流是一些症狀的統稱,典型症狀包括胸口悶痛、火燒心、胃酸返流、喉嚨異物感、慢性咳嗽等,這個病,基本上可以分成三大類:

  1. 食道損傷型逆流性食道炎
  2. 無損傷型胃食道逆流
  3. 神經功能過度敏感型

其中胃鏡檢查只能診斷第一種『損傷型』胃食道逆流,但臨床上有一半就診的患者,是屬於第二種或第三種,胃鏡檢查為正常,無食道損傷,但這些患者其實有典型症狀,所以仍稱之為胃食道逆流症,為了更精確的診斷,此時可以考慮進行食道酸鹼值的檢測,實際測量24小時或96小時食道是否有酸的反流,正常情況下食道屬中性,但當胃裡的胃酸逆流到食道時,食道的酸鹼值(PH)會下降到4以下,國際準則建議若一天內超過6%的時間出現胃酸逆流,或一天逆流的次數超過40次,則確定為『致病性的胃食道逆流症』,當然若胃酸逆流的比例低,則屬第三種神經功能過度敏感型,因此藉由食道酸鹼值的檢測,可以確認胃食道逆流的診斷,之後才能對症下藥,藥到病除。


李小姐:那我要做食道酸鹼值的檢測!要怎麼做呢?


醫師:食道酸鹼值檢測的方式可以分成兩種:有線式和無線式。

   『有線式』是從鼻子放入一根和鼻胃管相似的檢測管,深入胃部,測量管上分布著許多酸鹼值的檢點,可精準記錄24小時食道的酸鹼值,置入後回家,正常飲食,隔天回醫院由醫師將管子拔除,並進一步分析紀錄儀中酸鹼的數值。

   『無線式』則是利用胃鏡指引,在患者食道下端(胃頭上端6公分處)夾上一個特製的酸鹼值測量晶片,此微小晶片可以和記錄儀作藍芽比對,並記錄長達96小時的酸鹼數值,置入後患者舒適返家,可正常飲食並外出吃大餐,4天後將記錄儀繳回分析,晶片約一星期後自然掉落從糞便排除,不須撿回,因此是一種舒適度高,且檢查更精準客觀的方式。
